Chronic Care Consultants provides the services of licensed physicians for ongoing management of stable chronic conditions. To be eligible for participation in Chronic Care Consultants' programs, patients must have a pre-existing, diagnosed, documented chronic condition and treatment plan. Through the use of regular, remote-access care, Chronic Care Consultants can create a more efficient and convenient healthcare delivery system than is currently accessible to many patients.
