ER at Home is a Delaware company with a patient-facing mobile app that improves access to coordinated health care. We accomplish this through leveraging telehealth and tele-triage of patients and their health information to resource-appropriate services. We aim to be vertically aligned with all tiers of a community health infrastructure, with a particular focus on empowerment of patients and fortification of their relationships with PCPs. After providing better care for patients, our goal is to facilitate the appropriate utilization of emergency departments and hospitals. ER at Home will be a virtual, distributed network of health care providers linked by a common electronic platform that provides access and metrics that improve healthcare efficiency. The application has numerous value-added services such as granular tracking of health care performance metrics (clinical outcomes, compliance monitoring and patient satisfaction) as well as personalized health education, health tracking, positive behavioral reinforcement, appointment scheduling and an array of clinical trial support services.
