Chronic Care Consultants

Chronic Care Consultants provides the services of licensed physicians for ongoing management of stable chronic conditions. To be eligible for participation in Chronic Care Consultants’ programs, patients must have a pre-existing, diagnosed, documented chronic condition and treatment plan. Through the use of regular, remote-access care, Chronic Care Consultants can create a more efficient and convenient healthcare delivery system than is currently accessible to many patients.
